Client Registration & Request for Support Process

5 Steps for registering

Clear Links Support Ltd uses the term 'Client' to refer to the people we support.

Please Note: The Client Registration and Request for Support Form can be completed by you, or by a person on your behalf, for example a disability adviser at university or work colleague.

Below are the 5 Steps outlining the registration process and request for support for Service Users. If at any time you have any queries you can talk to someone during office hours on 0845 833 9971 (local rate number), 0114 278 6866 or you can email info@clear-links.co.uk

In accordance with UK Data Protection legislation, Clear Links Support Ltd will hold, process and use your data exclusively to provide you with the service for which you registered. Completing our registration form signifies consent to us collecting personal information. It also confirms your agreement that Clear Links Support Ltd may pass on relevant information to all those involved in the support work process.

Step One

You have been referred to us for specialist support for your education or employment.

Step Two

Please complete the Client Registration & Request for Support form. The form gives us the information about your required support. The form may be completed by you, or by another person on your behalf.

You should also read the 'Client Terms of Business' which constitute the contract between Clear Links and the client for the supply of the Support Worker’s Services by Clear Links to the client. They are deemed to be accepted by the Client when the Client registers on the Clear Links website, by virtue of the Client’s request for the use of a Support Worker, by the Client’s acceptance of the Support Worker’s Services or by the signing of a time sheet by the Client, whichever is the earliest. We encourage you to supply an email address when you register so that we can send you a link to some documents we expect you to read before support takes place.

Step Three

When we receive your completed Registration form and timetable we will store your details in our database. The information about your requirements is available to Clear Links Support Ltd office staff; the information is passed on to Support Workers on a 'need to know' basis. See Data Protection for more information.

Step Four

We will use your information to match you with a Support Worker. You will be contacted by Clear Links Support Ltd or the Support Worker to confirm the first meeting.
